How do I submit a specimen to CTA Pathology?

Specimens can be submitted via FedEx or UPS. Please address packages to 9750 SW Nimbus Ave, Beaverton, OR 97008. CTA Pathology can also provide shipping materials and labels; please contact our office if you would like to order supplies.

Depending on your location, we may also offer a courier service in your area. Please call 503-906-7300 to learn more about the courier options in your area.

What do I need to submit with the specimen?

We require all biopsy submissions to include a requisition which lists all relevant clinical information and patient demographics (including patient name, date of birth, address, and insurance information). Please also ensure that the specimen container has at least two identifiers listed (e.g., the patient’s name and date of birth) so we can match the requisition with the specimen.

How can I order supplies?

If you need more supplies, please email us at supplies@ctapathology.com with your clinic's name, address, and the specific materials needed. Depending on your location, you can expect supplies to either be dropped off by one of our couriers or shipped directly to your office. You are also welcome to call us and place your order over the PHONE.

How can I make an amendment to a pathology report?

If you need to make an amendment to a report, you can email our medical editing team at editors@ctapathology.com or call our office directly at (503) 906-7300 and request to speak with an editor.
